Hello,
You should be able to see all users databases, just your own.
If you're using "test" to login, then DA didn't create it because all users da creates should have username_name. If you're using the username for your DA account, then you'd be able to see all databases created by you.
If you can see the "mysql" database, then something is wrong.

Hello,
This should be done for you when it's installed by DA. Or just run:
cd /usr/local/directadmin/scripts
./phpMyAdmin.sh
to set it up again.

Hello,
http://www.directadmin.com
Are you a DirectAdmin customer? :) DirectAdmin (DA) is the software we sell which sets up all domains on a server. With the default install, it automatically installs all services, including apache, php, mysql, phpmyadmin etc.. and provides an easy means for setting them up.
:)
